The patient registration form has been redesigned. As of now, the diagnosis code (ICD) and the planned procedures (CHOP) are required when registering a patient. These are necessary for the calculation of the SwissDRG, the designation of the planned procedure in the operating room schedule, and for submitting the cost approval request to the health insurance provider.

In addition, for pediatric patients, the names of the parents must be stated. If a procedure on a paired organ is planned, please ensure that the corresponding side is clearly marked on the diagram provided. Finally, the patient registration form must be signed by you.
